Comments/Corrective Action Taken

Comments/Corrective Action Taken:

Kevin is the foreman and has the sssp with him, so it’s not on site today with Dale. Gave Dale a HBI ladder and MEPW inspection tag for him to use on ladders that our not ours. I went over the reasons and importance of inspecting equipment that is not our, and I took him to the closest Cochran ladder that was near us. That ladder had 4 bent rungs and a cut rung. Tried to find a Cochran employee to let them know but none were around. I explained to him that a simple check is all that’s needed before use. If that was our ladder I went over with him to danger tape it and bring it back to the warehouse for disposal. I could not locate the carpenter on site. Called him and called Juan with no response although he was seen earlier by Dale so I believe he is on site. I met with Eric the Sellen site superintendent and he informed me of their Monday safety huddles and break room availability for all crew members to take lunch with water provided. Standard ti pipe penetrations of ladders nothing was noticed as a safety concern.
