Comments/Corrective Action Taken

Comments/Corrective Action Taken:

Did safety walk with safety professionals from each trade and job mortinson. Job is in its early stages, but was not kelp up very well, hole covers were displaced or missing all over the job, concrete dust was left not cleaned up, with various other housekeeping issues. Ingress and egress was not marked, especially in an emergency situation this would be a significant problem. They had workers with no car zone for their lifts and no tool tethers there were rebar uncapped and poles sticking out of the concrete unmarked. Monoliths debris was everywhere from spraying. They did shut down the epoxy work to numerous complaints by workers. They will move this work to off hour work. Will go over with Erik. He is currently the only HBI employee on site. Mortinson does go over findings with entire crew at morning stretch and bend.
